It should be realised that many of these rhythms are not truly biological but are simply related to other relatively regular daily activities such as intake of food, differences in pos- ture, undertaking exercise, and the like, that is, pre- analytical influences rather than true biological variation. In addition to these daily and monthly cycles, some quantities have seasonal rhythms. Hardly surprising- ly, the concentrations of serum vitamin D metabolites have marked seasonal rhythms in individuals who live far from the equator since these quantities are sunlight dependent. There are other supposed sea- sonal rhythms, for example, serum cholesterol rises in the late autumn and falls in spring, and blood gly- cated haemoglobin rises in the summer. However, these again may not be truly biological but are likely related to other relatively regular daily events such as intake of food, differences in posture, and partaking of exercise. Thus, again, changes are pre-analytical rather than biological. After checking for sex-related differences in BV results, stratification by sex was justified for only five quantities (androstendione, estradiol, follicle stimulating hormone, luteinizing hormone and prolactin,). Two quantities (glucose and cholesterol) had to be stratified according to fasting or non-fasting state.

The assessment of biological variation, the natural fluctuation of body fluid constituents around the homeostatic setting point, is pivotal in clinical laboratories to set analytical quality specifications, as well as for evaluating the significance of changes in serial results (i.e., the reference change value, RCV) Laboratory diagnostics (i.e., the total testing process) develops conventionally through a virtual loop, originally referred to as ‘‘the brain to brain cycle’’ by George Lundberg. The RCV for interpreting a measured difference is based on the analytical imprecision as well as the CVwithinsubject estimated from healthy (or diseased) individuals during the steady state, assuming a gaussian distributions and homogeneity. For bio- logical data, inter-individual coefficient of variation (CVi) is often calculated after a log-normal transformation. It is important to emphasize that the RCV provides only a meas- ure for judging the probability that a difference in consecu- tive results can be explained by the analytical and CVws seen in patients in a stable situation. It does not provide a measure for judging the probability that a true change has occurred. Pre-analitik Analitik

This concept, originally proposed by Harris [9], provides an indisputable argument for a more appropriate use of population-based reference intervals, especially when monitoring individuals over time [10]. When the index is <0.6 (marked individual- ity), reference limits will be of limited use in evaluating a significant change. When the II is >1.4 (little individuality) it is accepted that the reference intervals will be satisfac- tory for monitoring for a significant change [7–12]. The ref- erence change value (RCV) takes analytical and BV into account and defines the percentage of change that should be exceeded, excluding the intrinsic variations that affect laboratory assays, in order to obtain a significant difference between two consecutive results [13, 14]. According to Ricós et al. [7], in most of the cases (except for the organ specific diseases markers), the RCVs in diseased individuals were of the same order as those in healthy individuals; small differences would have no effect on clinical applications. Our data indicated that for the investigated analytes, the RCVs of elderly people fall within the RCV ranges derived from the younger within-subject coefficients of BV, and they are similar to those observed in most clinical states. This fact suggests that the age-associated vulnerability and the changes in organ structure and function present in the elderly may be given the same consideration for dis- eases, and the use of specific RCVs is unnecessary.

Mevcut testler arası karşılaştırma yapılır. Testlerin klinik kullanılırlığı değerlendirilir. It is clear that clinical laboratory performance should satisfy medical needs (2), which include monitoring, screening, diagnosis and case finding. For monitoring a patient's condition, analytical variation has to be maintained below half the within-subject component of biological variation. This same specification has to be maintained for screening, diagnosis and case finding when a fixed cut-off point is used to define a pathological or healthy condition (3). To determine patient status for these three purposes according to population-based reference intervals, analytical bias must be maintained below a quarter of the within- plus between- subject components (4). Biological variation is a good basis for deriving analytical quality specifications that satisfy general medical needs.

40 While it is clear to everyone working in the field of labo- ratory medicine that most efforts have been focused on improving the quality in the analytical phase of testing, the high burden of errors still occurring within the preanalytical processes calls for a further broadening of the concept of TQM, to also embrace the processes external to the labora- tory, in an enterprise which would enable establishment of a governance of this often mistreated phase of the total testing process (8–10). ‘‘specimencare.com’’, is an online resource designed to identify, evaluate and promote the application of best practices in all aspects of the preanalytical phase of laboratory testing in clinical medicine.
