2 Giriş İnfertilite tüm dünyada evli çiftlerin en yaygın sorunu olarak bilinmektedir (%10- 15). Bu oranın %40-50si kadına ait faktörlerden kaynaklanmaktadır. Çalışmamızda kadın infertilitesinde klasik ovülasyon indüksiyonunu akupunktur ve düşük bazal insülin seviyesi elde eden beslenme düzeni ile destekleyerek gebelik oranlarını artırmayı hedefledik.

3 Yöntem 20-35 yaş arasında, tubal patoloji, erkek infertilitesi,yaygın endometrios içermeyen, jinekolojik değerlendirme ile ovulatuar patoloji saptanmış, 2 yıldan uzun süreli infertilitesi olan 58 kadın hasta üzerinde çalışıldı. Tüm hastalara TCM'ye göre hikâye, nabız, dil muayeneleri yapıldı. TCM ve jinekolojik kondisyonlarına göre 29 hastalık benzer iki grup oluşturuldu.Her hastaya en fazla 4 ay(siklus)olmak üzere , toplam 184 siklus ovülasyon indüksiyonu yapıldı. Birinci gruba sadece ovülasyon indüksiyonu uygulandı. İkinci grubun ovülasyon indüksiyonu ise, akupunktur ve beslenme ile desteklendi.

4 Sonuç Sadece ovülasyon indüksiyonu yapılan grupta siklus başına gebelik oranı %12,63 (95 siklusta 12 gebelik) iken, akupunktur ve beslenme düzeni ile desteklenen grupta gebelik oranı %19,10 (89 siklusta 17 gebelik) olarak saptandı.Hasta başına ençok 4 ay(siklus ) uygulandı.Sadece O.İ yapılan 29 hastadan 12si,O.İ+AKUPUNKTUR yapılan 29 hastadan 17si gebe kaldı.Gebelik oranı akupunktur uygulanan grupta %42 daha fazla saptandı.

11 Tartışma ve Sonuç Akupunktur ve beslenme desteği, infertilite tedavisinde klasik ovülasyon indüksiyonunun başarı şansını artırmakta ve yan etkilerini azaltmaktadır. Bunun nedeni vücutta endorfin, GnRH, gonadotropinlerin artışı veya dengelenmesidir. Akupunktur etkisi ile uterus, endometrium ve overlerin kan akımı artar. Ruhi ve bedeni dengesi, yıllarca süren stres, sıkıntı ve infertilite sebebi faktörlerle bozulmuş kadının, akupunktur tedavisi ile, ruh beden ve enerji dengesinin yeniden kurulmasının tedavi planlarının geleceğinde önemli bir yer tutacağı şüphesizdir.

12 Akupunktur Fertiliteyi Nasıl Arttırır?
Endokrin sistemi dengeleyerek. Pineal ve hipotalamus bezlerini direk etkileyerek gonadlar ve reproduktif sistem üzerine etki yapar. TCM’ye göre üreme böbrek, karaciğer, dalak, Chong ve Ren kanalları ile yakın ilişkilidir. Kan azlığına neden olan her şey infertiliteye sebep olabilir. 17 AETIOLOGY Constitutional weakness ( weakness of Kidney-Essence)
Overwork ( Kidney-Yindeficiency- basis for menstrual blood and the uterus ) Excessive physical work ( Weaken the spleen and Kidney Yang) Sexual activity at too early an age ( Weakens the kidneys and damages the directing (Ren Mai) and penetrating (Chong Mai) vessels and therefore cause infertility later in life ) Invasion of Cold(Internal cold obstructs the uterus and directing (Ren Mai) and penetrating (Chong Mai) preventing fertilization Irregular diet – iced execcive foods – cold in uterus Excessive greasy foods and dairy products. Dampness in the Lower Burner Dampness in the lower burner aesily turns into Damp- phlegm in which may obstruct the uterus preventing implantation of ovum.

18 Patterns of infertility
To get an idea of the statistical incidence of each type, we may refer to a study of 257 cases of infertility reported in Beijing's Journal of Chinese Medicene. Of these 257 cases, the following were the proportions of various patterns: Kidney-Yang deficiency: %. Spleen- and Kidney-Yang deficiency: 12.84%. Liver- and Kidney-Yin deficiency: 11.67%. Kidney-Yin deficiency with Empty-Heat: 5.06%. Liver-Qi stagnation: 7.39%. Blood stasis: 29.57%. Phlegm: 3.11%. Cold-Dampness: 2.73%.

19 Kidney-Yang deficiency
Clinical manifestations Prolonged menstrual cycle, the period could be either scanty or heavy, backache, dizziness, feeling cold, depression, frequent urination. Tongue: Pale, wet. Pulse: Deep, Weak. Treatment principle Tonify and warm Kidney-Yang, stoke up the Fire of the Gate of Life, strengthen the Uterus. Acupuncture KI-13, KI-3, BL-23, BL-52, Ren-4, Du-4, LU-7 andKI-6, Ren-8. All with reinforcing method; moxa should be used.

20 Kidney-Yin deficiency
Clinical manifestations Long-term infertility, periods early, scanty, with light-coloured blood, five-palm heat, night sweating, dizziness, tinnitus. Tongue: without coating; also Red if there is Empty-Heat. Pulse: Floating-Empty or Rapid-Fine (in case of Empty-Heat). Treatment principle Nourish Kidney-Yin and Kidney-Essence. Acupuncture Ren-4, Ren-7, KI-3, KI13, SP-6, LU7 and KI-6, BL-52. All with reinforcing method; generally speaking, no moxa should be used. However, if the tongue is not red or only slightly Red and the pulse is very Fine, a small amount of moxa (as a warm neddle) on KI-3 is beneficial to increase the tonifying effect.

21 Blood deficiency Clinical manifestations
Scanty periods, pale blood, delayed cycle, tiredness, depression, dizziness, pale complexion, blurred vision. Tongue: Pale and Thin. Pulse: Choppy or Fine. Treatment principle Nourish Blood and the Esserence, strengthen the Liver and Kidneys. Acupuncture Ren-4, ST-36, SP-6, KI-13, BL-20, BL-23, BL-17. All with reinforcing method, moxa is applicable.

22 Cold in Uterus Clinical manifestations
Primary infertility, delayed cycle, scanty period, small clots, painful period, better with heat, feeling colder during period, pale face, feeling cold, sore back. Tongue: Pale with thick, white coating. Pulse: Weak, Tight. Treatment principle Warm and tonify Kidney-Yang, warm the Uterus, scatter Cold. Acupuncture Ren-2, Ren-4, Du-4, KI-7, BL-23, Ren-7. All with reinforcing method; moxa should be used.

23 Dampness in the Lower Burner
Clinical manifestations Irregular periods, delayed cycle, mid-cycle pain, vaginal discharge, long-term infertility, adhesions, obesity, feeling of heaviness. Tongue: sticky coating. Pulse: Slippery. Treatment principle Resolve Dampness, remove obstructions from the Directing and Penetrating Vessels. Acupuncture Ren-2, ST-28, SP-9, SP-6, Ren-9, LU-7, ST-30, KI-14, BL- 32. All with reducing or even method. No moxa.

24 Stagnation of Qi Clinical manifestations
Irregular periods, pre-menstrual tension, painful periods, breast distention, irritability. Tongue: normal colour or slightly Red on the sides; if stagnation of Qi arises from Blood deficiency, the sides might be Pale. Pulse: Wiry, if Qi stagnation arises from Blood deficiency, the pulse may be Choppy or Fine in general and slightly Wiry on the left. Treatment principle Move Qi, eliminate stagnation, pacify the Liver, regulate the periods. Acupuncture LIV-3, G.B.-34, T.B.-6, P-6, Ren-6, Ren-4, KI-14, SP-4 and P-6. All with reducing or even method.

25 Damp-Phlegm Clinical manifestations
Scanty periods, a feeling of heaviness of the lower abdomen, obesity, excessive vaginal discharge, expectoration of sputum, pre-menstrual breast distension, swelling and pain. Tongue: Swollen, with sticky coating. Pulse: Slippery. Treatment principle Resolve Phlegm, tonify the Spleen and Kidneys. Acupuncture LU-7, KI-6, ST-28, Ren-4, KI-13, Ren-9, SP-9, ST-40, Ren-12, ST- 36, BL-20, BL-23. All with reducing or even method except Ren-4, KI-13, Ren-12, ST-36, BL-20 and BL-23 which should be needled with reinforcing method.

26 Stasis of Blood Clinical manifestations
Irregular and painful periods, dark blood with clots, irritability, mental restlessness, abdominal pain. Tongue: Purple. Pulse: Wiry or Choppy. Treatment principle Invigorate Blood, eliminate stasis, pacify the Liver and the Penetrating Vessel, Regulate the periods. Acupuncture LIV-3, G.B.-34, BL-17, SP-10, SP-6, P-6, T.B.-6, Ren-6, Ren-4, SP-4 and P-6, KI-14, ST-29, KI-6 and LU-7. All with reducing or even method.

27 Blood-Heat Clinical manifestations
Early periods (short cycle) up to twice a month, heavy flow, feeling hot during period, thirst mental restlessness. Tongue: Red with yellow coating. Pulse: Rapid, Overflowing. Treatment principle Cool Blood, regulate the periods. Acupuncture L.I.-11, SP-10, KI-2, SP-6, P-3, BL-17, Ren-4, LU-7 and KI-6. All with reducing or even method; no moxa.
